2024 Dental Open Enrollment Information

Union Fidelity Credit Union has agreed to continue taking our dental premiums from our members credit union account, at no charge to the participants. To have the United Healthcare Dental (DHMO), you must have an account at Union Fidelity Credit Union. If you need information on how to set up a credit union account, please call 713-869-9053. One of the requirements of Union Fidelity Credit Union is your account must have $100.00 in your account at all times (even after payments are drawn). Payments are taken from your Credit Union account for 3 months (quarterly) at a time (pricing listed below). The Credit Union takes the money out of your account on the 20th of every third month. Neither the Union Hall, nor the Credit Union, is responsible for notifying you if you are dropped from the dental plan due to insufficient funds.

 

For your United Healthcare Dental Insurance to be effective October 1, 2024, you must turn in the Dental Enrollment Form and Dental Deduction Authorization Form to IBEW Local Union 716 by Friday, September 13, 2024. Open enrollment is for all members of Local Union 716 and Retirees. If you are currently enrolled in the dental, you do not need to enroll again. If you are adding new dependents you need to enroll them during open enrollment. If you have been a previous participant and dropped from the dental, you will need to enroll again during this Open Enrollment.

 

The list of dental providers is available at the local union hall, or you may call 1-800-232-0990 and customer service can help you locate a dentist in your area, and give you the dentist providers number.

 

After the effective date of October 1, 2024, you will need to call 1-800-232-0990 to verify who you want as your provider and to make sure that is who they put you with. Also, this is United Healthcare’s customer service number if you need to change your provider.

 

Dental Plan Pricing

Prices are as follows effective October 1, 2024 thru September 2025.

  • Employee only: $46.62 (per quarter)
  • Employee + Spouse: $77.19 (per quarter)
  • Employee + Child(ren): $77.19 (per quarter)
  • Employee + Family: $113.67 (per quarter)
